This protocol is suitable for patients with or suspected of PE or DVT, with VA ECMO or needing anticoagulation for another reason.
Initial bolus is 80 units/kg to a maximumm of 10,000 units, calculated in Epic.
Initial dose is 18 units/kg/hr to a maximum of 2000 units/hr, calculated in Epic.
Subsequent goal aPTT is 60-85 seconds or heparin anti-Xa 0.3-0.7 units/ml.
The first aPTT is obtained 6 hours after initial infusion is started.
Once 2 consecutive aPTTs are between 60-85 seconds, repeat aPTT in am daily.
Notify physician for 2 consecutive aPTTs < 60 or > 85 seconds.
